Sports Accident Claims

Written notice of injury on which a claim may be based must be given to the Insurer within thirty (30) days after the date of the accident resulting in such injury.  Such notice must be given by or on behalf of the Insured Person or beneficiary, as the case may be, to the Insurer.  Information sufficient to identify the Insured Person such as a Policy Number and Policyholder Name must be provided.

Please follow procedures listed below in the event of a claim:

1. Complete a Claims Form.

2. Provide ORIGINAL receipts for all medical expenses, photocopies and carbon copies.
   
3. Mail all ORIGINAL receipts and completed Claims Form to the following address:
